How to Become a Maryland Designated
Yellow Fever Vaccination Physician
1. Mail a letter to Maryland Coordinator for International
Travel (see address below) requesting to become
a physician designated to provide yellow
fever vaccinations, along with the following:
- Physician’s Name;
- Practice Name (if applicable);
- Maryland State License (copy);
- Office Address;
- Telephone Number; and
- Fax Number
2. Upon verification that the requesting physician is in good
standing with the Maryland Board of Physician Quality Assurance,
the Maryland Coordinator for International Travel will mail an orientation
packet to the requesting physician containing the following:
- A Letter authorizing the requesting physician to secure a Uniform
Stamp for the validation of International
Certificates of Vaccinations (PHS-731).
- Yellow Fever Vaccine Information Statement (VIS)
- Yellow Fever Vaccine Storage and Handling Instructions
- International Certificate of Vaccination ordering process
- Yellow Fever Morbidity and Mortality Weekly Report (MMWR) Booklet
- Department of Health & Human Services Yellow Fever Update
- Yellow Fever Questionnaire
3. Upon receipt of the orientation packet, the physician should
have a Uniform Stamp created for the validation of International
Certificates of Vaccinations per the instructions in
the orientation packet.
4. The requesting physician should submit a letter, on the physician’s
letterhead, stating “This is the medical stamp for Dr..”
The letter should also include an imprint of the newly created Uniform
Stamp for the validation of International
Certificates of Vaccinations.
5. Upon receipt of the Uniform Stamp imprint, a letter of approval
will be sent to the requesting physician, Sanofi Pasteur Inc. (yellow
fever vaccine manufacturer), and the Division
of Global Migration and Quarantine of the Centers for
Disease Control and Prevention authorizing the physician to administer
yellow
fever vaccine.
6. Upon receipt of the approval letter the physician can begin
to administer yellow
fever vaccine.
7. Physicians designated to provide yellow fever vaccinations are
required to verify their continued usage of yellow
fever vaccine and the Uniform Stamp for the validation
of International Certificates of Vaccinations with the Maryland
Coordinator for International Travel on a biennial basis.
